For hepatic encephalopathy Xifapill works by inhibiting the growth of enteric ammonia-producing bacteria which are present in our gut, this results in the indirect reduction of serum ammonia level. In irritable bowel syndrome with diarrhea, how Xifapill works are still unknown but it is presumed that it works by changing the bacterial flora in small intestine and reduction in gas.
